If a seprate user was used for asm, then apparently the upgrade of the asm does not happen, even though no error is reported.
I could not find any reference to this problem in any Oracle (or other) document.

but that seems to be the opposite of my issues where it is showing the index tablespace rather than the old. the problem could be in the way a person uses these reports to debug

Agreed.
The behaviour of a read only database does not allow for passwords to be changedin physical standby;
Option #1 thus can only be used if the database is open for read write .

Ok, then check this out as regards your option 1:

Actually, you just need step 3 (which is copying over the file from primary to standby)...that should do it...even valid for 11g irrespective of SEC_CASE_SENSITIVE_LOGON setting.

This was not the case here since the primary database was on a HACMP cluster. broker was having difficulties between virtual nodes and physical nodes . oracle states that they do not support this type of config in 10 and in 11 only if clusterware is installed.

I got this same error once and in my case it was simply a case of the SYS password not being the same on the Standby as compared to the Primary. Once I got the passwordfile xfrd to the standby all went well :-)

Gary I just validated that a space management operation does not cause an implicit commit on the transaction. It seems oracle is calling an autonomous transaction to complete the drop which would not impact the transaction.

as far as what i drop this transaction did not commit probably becuase the drop trigger failed. good point though and chen shapiro has a post has implicit DDL commits

for a DDL before and after and in this case because of the drop table trigger the drop never happened and the insert never committed. i will have to create a testcase and see if a implicit drop table will cause a commit and this will be very concerning if it does.

Any way of telling whether that implicit drop table actually causes a commit in the primary transaction, or is it done in the same way as an autonomous transaction ?

RAT is a separate option, therefore it requires a license to be purchased.
